North Nassau Concierge Medical Care, founded by Dr. Eugene Raber, is a boutique-style medical practice emphasizing superb quality primary care services through a highly personalized relationship between doctor and patient.
It was created to provide individualized and tailored VIP-like medical care that caters to all the health needs of its members. The premise is on disease prevention and screening in addition to exceptional disease management. We integrate traditional and complementary aspects of medicine to provide members with the most up to date treatments in primary care.
As a board-certified geriatrician as well as an internist, Dr. Raber realizes the importance of keeping an elderly patient out of the hospital. The numerous risks associated with hospitalization, such as acquired infections, increased risk of confusion, bed sores/decubitus, and other complications, prompted Dr. Raber to implement the HOUSE CALL program. Elderly patients who were too frail or ill and were unable to travel to the doctor’s office in many instances ended up being unnecessarily hospitalized. In such situations, hospitalization could have been avoided if the patient was seen by a physician in a timely manner. This option is available to any member of the Nassau Concierge Medical Care requesting a house call.
Membership fees:
$1900 annually or $1000 semi-annually for home visits (extra charges may be included depending on travel time)
Memberships include:
60 minute — Comprehensive Annual Wellness Physical includes thorough history and examination, an extensive review of medical systems, comprehensive lab series and screening, resting, EKG, and spirometry. This enables us to establish a health baseline and create an individualized wellness program for your special needs (your insurance not billed)
30 minute follow up to review all results with a detailed explanation, identification, and discussion of health risk factors for major diseases and a customized treatment plan, prevention-wellness plan consultation (your insurance not billed).
Access via text or calls directly to Dr. Raber cell phone 24 hrs/day.
Same Day office visits (during regular office hours) and no wait time in the office for an appointment
Personalized coordination of care with all specialists and service
Guaranteed HOME VISITS same or next day (extra fee).
Ability to coordinate ancillary services including home blood collection and x-ray as well as assess need and implement a visiting home care nurse or physical therapist. (covered by medicare)
